New Patients

* I have reviewed the accepted insurances page and confirmed that my insurance is listed. I understand that I will enter my insurance information through the MyChart link.

* I am 16 years of age or older. Toothsuite is currently accepting new patients age 16 and older only.

* I have not had a dental cleaning within the past 12 months.

* I understand that all required paperwork must be completed through MyChart before my appointment request can be reviewed. Requests submitted without completed paperwork will not be approved.

* I understand that this is an appointment request, not a confirmed appointment. All requests are reviewed by Toothsuite, and I can expect an email update within approximately two weeks.
